In consideration of the benefits to be derived, and in view of the fact that the Proverbs 22 Classical Education Co-op is a non-profit organization, participation in which is wholly voluntary, and understanding that participation in our activities involves inherent risks and dangers, and in consideration of my child's being permitted to participate in the group’s activities, I, in my capacity as parent and legal guardian of the child/ren whose name/s is/are printed above, hereby:
(a) agree for my child/ren and/or myself to participate in group activities;
(b) agree to assume all risks associated with such activities;
(c) understand that aside from designated class times, I am solely responsible for the safety and well-being of my children while at co-op;
(d) certify that I am legally allowed around children and have not been convicted of or currently involved in any case regarding any crime involving children, drugs, or violent acts;
(e) release, forever discharge, and covenant not to sue Proverbs 22 Classical Education Co-op, including members of the group, and all other affiliated or associated organizations or entities, Compassion Church (at 3525 US-70, Dickson, TN), volunteers, teachers, and other children (collectively, the "Released Parties"), from and/or based on any and all claims, demands, rights of action or liabilities of whatsoever nature, including but not limited to claims based on negligence, whether known or unknown, which my child or I had, now have, or may or might have in the future, based upon, arising out of, or in any manner connected with my child's and/or my participation in the group’s activities, except that none of the Released Parties are released from liability for injury, loss, or damage caused by his/her/its own recklessness or willful misconduct.
Furthermore, by my signature below, I also sign for my spouse. I agree to indemnify and hold harmless the Released Parties from any expense, including attorneys' fees (including the cost of defending any claim released by this agreement), as to any and all claims released under this agreement on behalf of my child and/or myself. This instrument shall be governed by Tennessee law, notwithstanding any state's choice of law rules to the contrary.
